April is STD Awareness Month. Despite the many efforts of safe sex messaging, nearly 20 million new sexually transmitted diseases (STDs) occur in the United States each year.

When you think of safe sex and STD’s, rarely do you think about oral health. As a dentist that goes the extra mile by checking for oral infections beyond cavities and gum disease, you’d be surprised to know how often I find STD’s such as herpes both inside and outside the mouth. Herpes lesions are usually painful, unsightly, and most importantly contagious. When in the mouth, STD’s can affect your ability to talk, eat, and overall interrupt your day. Today I am sharing how recognize the signs and symptoms of herpes infections so that you can seek treatment and prevent infecting others during intimate contact.

Herpes and The Mouth

Herpes is one of the most common and infectious STD’s that may be discovered by your dental professional. There are two types of herpes-oral (herpes simplex virus 1) and genital (herpes simplex 2). Herpes simplex virus 1 causes 95-97% of oral cold sores. Herpes is usually spread by kissing someone who suffers from the herpes virus in or around the mouth (cold sores), receiving oral sex from someone who has oral herpes, and of course traditional genital sexual intercourse that is not protected. Oral herpes is 6 times more infectious on the mouth than when it is on the genitals. As a matter of fact, oral herpes causes 50-70% of new genital herpes infections. Hence, oral herpes is not to be taken lightly! The tricky thing with herpes is that it is very contagious- even when symptoms are not present. If you suffer from oral herpes (cold sores) or genital herpes, it’s extremely important to disclose your medical history and take the necessary precautions when engaging in oral or genital sexual intercourse.

Common Signs and Symptoms of Oral Herpes

Common warning signs that occur before the sores actually appear include tingling, pain, burning sensation, or itching at the site of infection. In addition, you may develop a fever, feel more tired, develop muscle aches, and may feel overall more irritable. This known as the prodrome phase. Stress is often a factor that can spark a herpes outbreak.
When the sores finally appear, they will usually appear initially as clusters of blisters located outside the lips, gums, tongue, roof of the mouth, and inside the cheeks. These blisters will then burst and turn into painful ulcers with a red base until they dry up and finally dissolve.

Treatment and Prevention of Herpes Infections

If you suffer from symptoms of herpes and think you may have been exposed, set up an appointment with your dentist and/or doctor for an immediate evaluation. The distinct appearance of herpes makes it easy to identify visually, however blood tests and viral cultures will confirm herpes diagnosis. The herpes virus does not have a cure, but you can control your outbreak symptoms, treat the sores and ulcers, and manage the frequency of outbreaks.

Prescription oral medications: Valtrex and Zovirax are popular anti-viral pills used to treat both genital and oral herpes. These can be taken right at the onset of an outbreak to shorten the length of symptoms or taken daily to manage the frequency of outbreaks. Zovirax also comes as a prescription cream that can be applied directly on herpes sores outside the mouth for relief.

Over the counter medications: Abreva is the only non-prescription ointment for oral herpes that has been FDA-approved. Over-the-counter topical anesthetics such as Ambesol gel and Blistex lip ointment may also be applied directly on the sore to provide modest relief. Tylenol and Motrin are good choices for quick and effective pain and fever relief.

Condoms and Rubber Dams: Both are recommended to help prevent herpes transmission during genital and oral sex. The goal is to avoid touching saliva, skin, or mucous membranes of people who have herpes symptoms or lesions but protection is never 100%. Unlike condoms, rubber dams are not commonly found in our local pharmacies and grocery stores to use while performing oral sex on women. I would recommend contacting your local Planned Parenthood or Department of Health for rubber dams. Private sellers and businesses specializing in safer sex supplies are also another option. In New York City, Babeland sells dental dams for just fewer than two dollars, and also provides confidential online shopping and shipping services. Condomania and Good Vibrations also sell dental dams online.

March 26^th, 2013 is Diabetes Alert Day. American Diabetes Association® Alert Day® is a one-day “wake-up call” asking the American public to increase their diabetes awareness and check their own health by taking the Diabetes Risk Test to discover if they are at risk for developing type 2 diabetes.

Just 1 month ago I decided to practice what I preach and take charge of my health. This included visits to my primary care physician, OBGYN, my own dentist (yes, I actually go to the dentist), and finally my favorite nutritionist Dr. Jairo Rodriguez. It was during this overdue trip to Dr. Rodriguez that I discovered that not only do I have high cholesterol; I am also at risk for diabetes!

What is Diabetes?

Diabetes is simply having too much sugar in the blood. When we eat, our body breaks down food into sugar and sends it through our bloodstream. The pancreas then makes a hormone called insulin. Insulin’s job is to take the sugar in our blood and feed our cells to be used for the energy we need for daily life. That’s right, we need sugar for energy. However, people who have type 2 diabetes either do not make enough insulin on their own or the insulin that their body makes doesn’t work well to control sugar levels. Type 2 diabetes can be determined by testing your blood to see how much sugar is in your blood. Too much sugar in the blood can mean that you have diabetes. Prediabetes is when a person has high sugar in their blood, but not enough to be called diabetes. People who are pre-diabetic can change their diet and exercise to delay or prevent diabetes. My diagnosis is prediabetes, which means that I’ve got to make some IMMEDIATE changes in my life so that I can remain healthy. It’s a condition that can take a serious toll on your body—including your dental health.

Diabetes and Your Dental Health

There are 26 million people with diabetes in the United States alone and 95% suffer from mild, moderate, or advanced gum disease. It’s important for the public to know that there is a strong connection between diabetes and gum disease. People who have uncontrolled diabetes are more inclined to have gum disease. High blood sugar is the perfect breeding ground for bacteria and infections to grow all over the body, which includes infections in the mouth. A gum infection in the mouth in turn interferes with a diabetic’s body to use insulin to control blood sugar. The two truly go hand in hand to wreck havoc on the body.

The common signs and symptoms of diabetes in the mouth may include:

Bleeding gums
Loss of gum tissue and supporting fibers
Loose teeth
Persistent bad breath
Dry mouth
Tooth decay
Yeast infection in mouth (Thrush)

Patients like myself who are pre-diabetic, diabetic, or have a family history of diabetes and are predisposed to developing diabetes should seek the advice of their family physician and dentist. These two health care providers should be an integral part of your health team and will need to work together, especially if you require advanced dental care. The average diabetic spends $1,800 a year on gum disease treatments as a complication of their diabetes.

Helpful Tips To Control Gum Disease and Diabetes

The most important thing that a diabetic patient can do is practice good oral hygiene habits at home to reduce infection.

Brushing, flossing, and use mouth rinse 2-3 times a day to keep bacterial levels down. Consider investing in an electric toothbrush which is more effective in cleaning than manual toothbrushes.
Contact your dentist or hygienist if you experience any of these signs of gum disease: Gums that bleed or are red, puffy or swollen, or sore; Gums that have pulled away from your teeth; Changes in the way your teeth fit together when you bite; Pus that appears between your teeth and gums; Or constant bad breath or a bad taste in your mouth. A diabetic patient may likely need to make more frequent visits to the dentist for preventive care (every 3-4 months)
When making food choices read labels. Consider the sugar content and the number of times a day you eat sugary snacks, foods, and beverages.

If you are a diabetic or are at risk for developing diabetes, these tips can not only save you money in dental and medical bills, but also save your life. Dislodged or Misaligned teeth. Dislodged or misaligned permanent teeth may require costly braces to correct the alignment. If nerve damage occurs, root canal treatment will also be required.
Knocked-out teeth. If a tooth is completely knocked out, you must immediately save and store the tooth in milk or saline and seek dental/medical care immediately to put the tooth back in its socket.
A broken jaw. A broken jaw must be set back into its proper position and stabilized with wires as it heals (six weeks or more, depending on your age and severity of fracture). An oral/maxillofacial surgeon usually performs this procedure.

CHOOSING THE BEST MOUTH GUARD

There are two types of mouth guards. Over the counter pre-made mouth guards and custom made mouth guards, which you order at a dentist’s office. Custom mouth guards are highly recommended because they are customized to fit the exact shape of your child’s teeth to provide more protection and are much more comfortable. The store bought mouth guards do help protect a child’s teeth to a certain extent, but do not offer the custom fit which can lead to dislodging, gagging or difficulty breathing, and just feel uncomfortable. If you are not sure which type of mouth guard is best for your child and his/her activities of choice, consult with your dentist. If choosing a store bought guard, choose one preferably rated well by the American Dental Association (ADA).

THE COST OF NOT PROTECTING YOUR CHILD’S TEETH

Pre-made mouth guards can range from $15-$30. Custom made mouth guards made by your dentist can range from $100-$300. Before thinking about the cost of investing in a custom made or store bought mouth guard, as a parent you may want to think about the cost of NOT investing. Repairing sports related tooth injuries, especially without dental insurance can definitely disrupt the family budget. The cost of replacing a missing tooth with a bridge or implant could cost $2000-$3000 each; the cost of repairing a cracked tooth with bonding can range from $250-$500/tooth; root canals can range from $900-$1200 per tooth; and porcelain crowns can range $800-$1700 depending on what region of the country you live and if you have insurance or not.

Anguilla 2012: First Rick Ross, then the Real Housewives of Atlanta, and finally the return of Dr. Catrise Austin

Anyone who knows me knows that I love and have a passion for 4 things in life: dentistry, music and the arts, food, and finally travel. I can’t tell you which one I love the most, I just know that I can’t live without any of them.

Now if you follow me on Twitter (www.twitter.com/drcatriseaustin) you would know that I’ve been a little upset lately because when I find a precious jewel I like to keep a secret, untainted. And that’s what I considered the island of Anguilla when I first stumbled upon it in 2011 as we docked at St. Maarten during the Tom Joyner Fantastic Voyage Cruise. The reason why my heart is a tad bit broken is because this year there’s been an invasion of Hip Hop stars and reality TV shows like The Real Housewives of Atlanta and I fear that this precious jewel of an island could lose that very thing that makes it special, its serenity.

I’d been to St. Maarten several times so when the Tom Joyner Fantastic Voyage docked at the port I wanted to do something different and explore somewhere I had never been during the short six hour stay. My pick was the celebrity favorite, the island of St. Barth’s. Unfortunately by the time I got off the cruise ship, the boat to St. Barth’s had already taken off so I was left with no other choice (if I was going to explore something different) then to catch the next boat to the island of Anguilla. “What do I have to lose, we are in the Caribbean” I said to myself. I no idea of the beauty, splendor, the kind people, and adventure that awaited me.

My girl Kyla was adventurous enough to explore with me. When we arrived, we were told that we should either visit the fabulous Viceroy Hotel for food and drinks or we could chill at the beach that had been consistently voted one of the top beaches not only in the Caribbean, but in the world- Shoal Bay. The
clock was ticking so we told our taxi driver, “Take us to Shoal Bay as quickly as possible”!

When we arrived to Shoal Bay we lucked upon the perfect spot, with the perfect hosts. If you remembered earlier I mentioned that I have a passion for food so my first mission was to find a spot where I could get fresh caught red snapper, rice and beans, and a nice rum punch to wash it down. We lucked upon a hut at the end of Shoal Bay where we met Carol who was the sweetest, most accommodating host. She answered our every call with the freshest snapper, the tastiest rice and beans, and I swear to you the best rum punch that I’ve ever had in my entire life!

After having a great meal and a little bit of sun tanning, the clock was ticking but it was time to have more fun. Along came Junior (the famous Junior that is) who owns the glass bottom boat. After feeling the warm Anguilla sun on our bodies we thought “perfect time for boat ride and to play in the ocean”! The cool Caribbean water, the fish that we saw through the boat’s glass floor and ride was simply amazing. If only we have more time to enjoy and explore. Time was up and we had to go back to St. Maarten to catch our cruise ship. I vowed not only to return, but to return soon as possible!

My opportunity came again this year when I was invited as an author to attend The Anguilla Literary Festival by my dear friend and book dealer Marva Allen, Owner of Hue-Man Bookstore (www.huemanbookstore.com). Authors such as Hill Harper, Sheryl Lee Ralph, Terrie McMillan, Crystal McCrary, and more attended this intimate festival. What an honor for me to be in the presence of thinkers and literary masters.

During the opening reception which was held at The ViceRoy Hotel, I met a great couple Gerry and his wife who had moved from the US to Anguilla a few years back. Gerry has personality for days! He and his wife instantly took me in and graciously became my companions and hosts for the weekends.

For the next 3 days the conference was packed with wonderful panel discussions about writing, culture, and included great readings by famous and not so famous authors. On the final day, we played hooky from conference activities and decided to take a day trip to Shoal Bay. Gerry picked me up and brought me to his home so we could pick up the rest of the family which included his wife, dog and pet parrot! Yes, we ALL were going to the beach as one BIG happy family. I called my old friend Junior to let him know that I was back “home” and asked if we could meet up for a day in the water. Did I fail to mention, I also love all things water. You see, I grew up boating and fishing in the great lakes of Michigan and I also used to swim darn near everyday in middle school. Til this day, I swim at least twice weekly here in NYC…I love water! After visiting Carol again for rum punches and having crabs with my Gerry and the gang, Junior and I went for a snorkel that was just a splendor of beauty. If only I could have the ocean as my playground like Junior. After my snorkel, I soaked up some sun with an unlikely companion, Gerry’s pet parrot!

Once again I left Anguilla wanting more. I thought how lucky I am to have been invited back for business and to leave with even more friends on this exclusive island. But will this island remain exclusive despite the flashing lights of videos now being made there like Rick Ross’s “Diced Pineapples” or reality shows like the 3 part series of The Real Housewives of Atlanta in Anguilla? I certainly hope so! I guess that I shouldn’t be so selfish when it comes to Anguilla, but when you find a good thing you want to hold on to it. I just want to see the place I’ve come to grow and love remain quiet, peaceful, and let its beauty continue to shine because Anguilla truly makes me smile!
